TV networks got shootings wrong

With respect to the Sandy Hook school shootings, how could all the major TV media get so much factually wrong, so quickly?
This problem is not isolated to any one organization, it spans all networks. This ready, fire, aim mentality and the use of “anonymous sources” needs to be seriously questioned.
